Job summary

The Elizabeth Layton Center, Inc. in Paola, KS is seeking an IPS Supported Employment Case Manager to join our outpatient team. You will provide case management to consumers with mental illness seeking stable employment and connect them to employment opportunities.

Work with therapeutic staff to improve independence, productivity, and community inclusion; coordinate with employment agencies and social services; perform on-site and community-based services within consumer settings.

Qualifications

• Bachelor’s degree or equivalent experience to meet State of Kansas licensing regulations.
• Familiarity with mental health theory and practice.
• Clean driving record.
• Incumbent must pass a security check as required by Medicaid Regulations.

Responsibilities

• Facilitates care coordination between client and local businesses to obtain employment.
• Builds and maintains positive relationships with local businesses and social services partners.
• Provides individual vocational services: applications, resume writing, interview skills, and employment readiness.
• Attends weekly individual and group supervisions.
• Meets with vocational specialists and completes required documentation.
• Works with treatment teams to support client symptom self-management for employment goals.
