Job summary

Qualifications

• Bachelor degree or LPN certification in related fields as stated.
• Strong writing, communication and analytic abilities are required.
• Valid Ohio driver's license and access to insured vehicle for work travel.
• Ability to obtain Ohio DODD Investigative Agent certification.

Responsibilities

• Investigate, report and follow up on Major Unusual Incident (MUI) information per DODD rules and LCBDD policies.
• Conduct interviews within required timeframes and gather documentation to identify causes.
• Write thorough reports using DODD-prescribed formats.
• Provide oversight and analysis of provider documentation per Ohio Administrative Code.
• Deliver annual and initial trainings and ongoing support for providers and LCBDD staff.
• Collaborate with team to develop and implement prevention plans to reduce incident recurrence.
• Create/update forms and documents used by Investigative Agents.
