Qualifications

• 3-5 years nursing experience in inpatient or ambulatory settings with complex medical or surgical patients
• Oregon RN license required or eligible to obtain prior to start date
• Strong assessment, triage, and care coordination abilities

Responsibilities

• Deliver direct and indirect patient care within RN scope
• Provide patient education and counseling tailored to needs
• Triage telephone and in-person patient inquiries and coordinate care
• Review patient charts prior to provider visits and prepare for visits
• Coordinate with medical assistants to set up labs and tests
• Assist with prior authorizations and documentation as needed
• Communicate effectively with providers, patients and staff using Epic/Outlook
