• Current BLS certification.
• Experience on a hospital unit and discharge planning exposure.
• Ability to perform duties per SOPs and policies.

Responsibilities

• Perform daily rounds on all patients and coordinate care.
• Develop care plans based on assessments considering patient needs.
• Assess conditions, set goals, and prescribe nursing actions.
• Address patient self-reported pain promptly per SOPs.
• Identify early warning signs and respond to deteriorations.
• Assist with emergency measures as needed.
• Document observations accurately for the team.
• Initiate discharge planning and patient education early.
