Qualifications

• Associates Degree in Nursing required.
• New Hampshire RN license required.
• Minimum of 3 years of relevant RN experience required.
• BLS certification required within 30 days of the beginning of employment.

Responsibilities

• Coordinate care and schedule exam preparation and paperwork.
• Conduct outreach to referrers, providers, and other medical professionals.
• Act as educator and advocate for patients from entry through diagnosis and treatment plan.
• Build relationships with patients and physicians to coordinate care including appointments, transportation, education, and support services.
• Serve as liaison between patient, family, primary care physician, internal and external care providers, and the broader healthcare team.
• Collaborates with radiologists and primary care physicians to deliver multidisciplinary care.
• Manage flow of patients including screening, prep, nursing care, and arranging testing and admissions.
• Instruct patients and families regarding medications and treatment instructions.
• Administer medications per physician orders.
