Job summary

Lifekind Health in Victorville, CA is seeking an Interventional Care Coordinator to support a medically complex patient population. You will document encounters, manage scheduling, and coordinate with medical and behavioral providers to ensure seamless care.

Responsibilities

• Care for a medically complex and emotionally challenging patient population with compassion and deescalation skills.
• Document medical and behavioral encounters in health record management systems.
• Administer and process consent forms and other patient questionnaires as needed.
• Assist medical and behavioral providers in writing and transferring orders.
• Process imaging orders and transfer imaging to health records.
• Prepare patient charts prior to encounters and ensure records are complete.
• Collect vital signs and initial screening information; administer urine drug screens and document results.
• Review CURES reports with medical providers as needed.
• Process referrals to external providers and resources.
• Document injections, prep, and assist with injections.
• Coordinate scheduling, confirmations, and medical records requests; follow up on appeals/denials.
• Maintain clean exam rooms and stock supplies; notify supervisor when supplies are low.
• Comply with applicable laws and regulations; respond to patient communications.
• Process lab orders and complete medical clearances for procedures.
• Maintain Excel spreadsheets for procedure readiness.

Job Duties

• Care for a medically complex and emotionally challenging patient population that requires compassion, empathy, and the ability to deescalate and appropriately react to unacceptable behavior in individual and group settings.
• Document medical and behavioral encounters in health record management systems (prep, complete patient record with Dx, Hx, care plan, CC/HPI, billing info, etc.).
• Administer and process consent forms and other patient questionnaires as needed.
• Assist medical and behavioral provider in writing and transferring orders.
• Process Imaging orders as per provider request (complete forms, send tasks & document), transfer imaging from partner portals into health record management systems.
• Prepare patient charts prior to medical and behavioral encounters (upload orders, referrals, imaging, etc.).
• Collect and document vital signs, Urine Drug Screen, and initial patient screening information (Temperature, COVID forms, etc.).
• Administer urine drug screen and record results. Assist medical provider with UDS interpretation and process UDS confirmation as necessary.
• Obtain and assist medical provider in reviewing the Controlled Substance Utilization Review and Evaluation System (CURES) report.
• Process referrals as ordered to other external providers and community health resources.
• Document injection consents, prep and assist with injections.
• Provide patient support in the clinic including: schedule appointments, enter authorization dates, appointment confirmation calls, medical records requests and follow up on appeals and/or denials.
• Ensure exam rooms are clean and stocked. Notifies supervisor when supplies are needed.
• Comply with federal, state, and local legal and professional requirements.
• Respond to patient phone calls, texts & voicemails from Medical Team Unit phone. Make calls as per assigned duties (pharmacy, appointment confirmation, etc.).
• Process lab orders (complete forms, send tasks & document).
• Complete Medical Clearances as needed for patient procedures.
• Update and maintain an Excel Spreadsheet to ensure patients who meet the requirements for Advanced Procedures are prepared for their procedure (Psych & Cardio clearance, medical records, imaging, etc.)
• Scheduling of Interventional Pain procedures at the Golden Springs Surgical Center (Procedure, Post Op, Follow Up, etc.)
• Personal Injury patient follow up for appointments, calls to reschedule and follow-up with patient and legal representatives.
• Performs other duties consistent with the above responsibilities.
