Job summary

Lifekind Health in Riverside, CA is seeking a Care Coordinator to join our team. You will assess patient needs, develop care plans with clinicians, coordinate appointments, and serve as the primary contact for patients and families.

The ideal candidate has at least 2 years in care coordination, knowledge of healthcare systems, strong communication, and EHR proficiency. This full-time role requires weekend availability and a dedication to reducing barriers to care.

Qualifications

• At least 2 years of experience in care coordination, case management, or a related healthcare role.
• Certification in Medical Assisting (MA), Case Management (CCM) or CCC is a strong plus.
• High School Diploma or equivalent required.
• Strong knowledge of healthcare systems, patient care processes, and community resources.
• Proficiency in electronic health records (EHR) and basic computer applications.
• Bilingual abilities to support non-English speaking patients preferred.

Responsibilities

• Assess patient needs and develop individualized care plans in collaboration with healthcare professionals.
• Coordinate appointments, treatments, and follow-up care to ensure continuity and adherence to care plans.
• Serve as the primary point of contact for patients and families, providing education and support throughout the care process.
• Facilitate communication between patients, healthcare providers, and community resources to address social determinants of health.
• Monitor patient progress and update care plans as necessary, documenting all interactions and outcomes accurately.
• Identify and address potential barriers to care, including transportation, financial constraints, and language differences.
• Maintain compliance with healthcare regulations, privacy laws, and organizational policies.
• Participate in interdisciplinary team meetings to discuss patient care strategies and improve service delivery.

Schedule : Full -time, 5 days per week. Clinics are open 7 days a week, 8:00 am - 7:00 pm. Availability to work Saturday and/or Sunday is required

Our Ideal Candidate will have the following qualifications:

• At least 2 years of experience in care coordination, case management, or a related healthcare role
• Certification in Medical Assisting (MA), Case Management (CCM) or Certified Care Coordinator (CCC) credential a strong plus
• High School Diploma or equivalent required
• Strong knowledge of healthcare systems, patient care processes, and community resources
• Excellent communication and interpersonal skills to effectively interact with diverse patient populations and healthcare teams
• Proficiencyin electronic health records (EHR) and basic computer applications
• Experience working with vulnerable populations, including elderly or chronically ill patients preferred
• Basic Life Support (BLS) or CPR certification is a plus
• Bilingual abilities to support non-English speaking patients preferred
