Wake County EMS is seeking CORETeam Specialist professionals to support the Advanced Practice Paramedic program. The role focuses on identifying high utilizers, connecting them with resources, and coordinating care across the healthcare continuum to improve community health.

Qualifications

• Associates degree in EMS or healthcare related field OR social work or a related field.
• Three years cleared as a Paramedic in the Wake County EMS System OR demonstrated experience in clinical social work or an adjacent field
• Certification as a North Carolina Paramedic or eligibility for such certification
• Valid Driver's License and "safe" driving record
• Equivalent education and experience are accepted

The Crisis, Outreach, Reduction, and Engagement (CORE) Team Specialist plays a vital role within the Wake County EMS Advanced Practice Paramedic program. CORE Specialists ensure that the highest utilizers of EMS services are identified, connected with appropriate resources and aligned with care that meets their unique needs across the larger healthcare continuum. About Our Team

Wake County EMS is a nationally recognized leader in EMS and prehospital medicine. The Advanced Practice Paramedic program serves Wake County residents and visitors by linking them with alternatives to traditional transport to the emergency department including mental health care access, opioid overdose intervention and falls assessment. Additionally, the Advanced Practice Paramedics respond to high acuity incidents to serve as a leader and resource for EMS crews on scene. Wake County EMS responded to over 140,000 requests for 911 EMS services in 2025. Beyond the Basics (Preferred Education and Experience)

• Bachelor’s degree in Emergency Medical Services or related field OR social work or a related field
• Current experience as an Advanced Practice Paramedic

How Will We Know You're 'The One'?

• Proficiency in working with Salesforce and/or case management agencies
• Ability to connect, engage and meet with high volume utilizers and other specialty patients to provide patient education directly related to medical conditions and social determinants of health
• Ability to apply advanced knowledge of social determinants of health to identify gaps and barriers affecting a patient's overall quality of life and their need for the EMS System
• Ability to complete documentation through patient care reporting systems, electronic logs, reporting software and databases
• Ability to maintain confidentiality and protect sensitive data, in accordance with HIPAA laws, and organizational and departmental policies and procedures
• Ability to collaborate with community partners to evaluate any specialty treatment and/or care plans for high volume utilizers to reduce EMS and hospital utilization
• Ability to participate in meetings when required with partner agencies and provide professional contributions and feedback
• Ability to responds to EMS calls when requested and evaluate patient conditions based on criteria and advanced decision-making skills and possible alternative transport to an alternative medical care facility
• Ability to draft patient specific destination and treatment plans to align community collaborative efforts
• Ability to facilitate meetings with the Division of Medical Affairs, sharing drafted destination and treatment plans

Hiring Range: 36.75 - 47.78
