Job Juncture in Illinois is seeking a full-time, in-person Internal Medicine Physician to join a mission-driven outpatient practice serving diverse, underserved patients. You will provide comprehensive outpatient care in a collaborative team with emphasis on prevention, chronic disease management, and community health impact.

Qualifications include MD/DO, Board eligibility/certification, active license, and ability to obtain; strong clinical judgment, communication, and team approach.

Qualifications

• MD or DO from an accredited medical school.
• Board Eligible or Board Certified in Internal Medicine.
• Active state medical license (or ability to obtain).
• Experience in outpatient or community-based care preferred.

Responsibilities

• Provide comprehensive outpatient internal medicine care to adult patients.
• Diagnose, treat, and manage acute and chronic medical conditions.
• Deliver preventive care, health screenings, immunizations, and wellness services.
• Develop and manage individualized treatment plans and follow-up care.
• Collaborate with interdisciplinary clinical teams and coordinate specialty referrals.
• Deliver culturally competent care with consideration of social determinants of health.
• Participate in quality improvement initiatives, care coordination, and population health efforts.
• Accurately document patient encounters in the EHR.
• Engage in clinical meetings, care team collaboration, and community health initiatives.

Compensation: $200,000 – $295,000 per year
