Qualifications

• Requires Bachelor's Degree in Pharmacy with valid MA license.
• PGY1/PGY2 training preferred.
• Inpatient/central pharmacy experience is advantageous.

Responsibilities

• Review and verify medication orders and patient charts for safety and appropriateness.
• Collaborate with central pharmacist and Purchasing on medication needs.
• Provide transitions of care, including reconciliation and discharge counseling.
• Serve as a preceptor for residents and students.
• Participate in formulary management and drug shortage actions.
• Lead quality improvement projects to enhance patient care.
