Qualifications

• Requires a Degree in Pharmacy from an accredited college of pharmacy.
• Massachusetts pharmacist license in good standing by hire date is required.
• Board of Pharmacy Specialties (BPS) certification OR CCP certification (except BLS/ACLS/PALS) or obtained after start date.

Responsibilities

• Monitor patients for appropriateness and response to therapy.
• Document pertinent findings in the electronic medical record.
• Develop, document, and execute patient-centered care plans using guidelines or protocols.
• Provide ongoing care for chronic conditions per protocol.
• Order laboratory studies and analyze results to adjust therapy.
• Provide drug education and counseling to patients.
• Serve as a drug expert and mentor to staff and students.
