Job summary

Qualifications

Responsibilities

• Assess patients’ physical, social, and psychological status using the nursing process.
• Collaboratively plan individualized care with the interdisciplinary team and patient/family.
• Implement and coordinate care with the care team, ensuring timely documentation.
• Evaluate patient outcomes and update the plan of care as needed.
