Job summary

DuPagePads is seeking a Housing & Case Management Coordinator to provide assessment, case management, and referral services for the Interim Housing program, advocating for clients and coordinating with community resources.

This full-time, non-exempt role requires strong organizational skills, crisis intervention ability, and excellent communication to support housing stability and program outcomes.

Qualifications

• Bachelor's degree in social work or related field.
• Experience in case management.
• Experience or knowledge of homelessness, poverty, and mental health preferred.

Responsibilities

• Manage a caseload of assigned clients.
• Screen and assess participants in Interim Housing programs.
• Provide individual case management to establish housing stability goals.
• Work with clients to develop transition and employment plans.
• Provide referrals to community services.
• Provide support and enrichment programs for clients.
• Advocate for increased community education and resources.
• Teach independent living skills.
• Attend required staff and supervision meetings.
• General front-office tasks and documentation duties.
