Job summary

Community Medical Center in Missoula, MT, part of Lifepoint Health, seeks an Interim Director (RN) Case Management to lead the team and drive seamless patient care from admission to disposition.

You will oversee care coordination, discharge planning, utilization review, and resource management, partner with physicians and nursing leadership, monitor KPIs, ensure regulatory compliance, and advance initiatives to elevate patient experience.

Qualifications

• Current Montana RN license or Compact RN License with Montana endorsement
• Bachelor's Degree in Nursing (BSN) from an accredited nursing school
• Education: Master's degree (nursing or relevant field) preferred
• Certification: Basic Life Support (BLS) required
• Experience: Minimum 1 year Hospital Case Management; minimum 2 years leadership experience preferred

Responsibilities

• Lead and develop the Case Management team with a culture of collaboration and accountability
• Oversee care coordination, discharge planning, utilization review, and resource management processes
• Partner with physicians, nursing leadership, and interdisciplinary teams to support patient progression
• Monitor KPIs, identify improvement opportunities, and implement strategies for better outcomes

How you'll contribute

What we're looking for

Applicants should have a current Montana RN license or Compact RN License with Montana endorsement, and possess an Bachelor's Degree in Nursing (BSN) from an accredited nursing school. Additional requirements include:

• Education: Master's degree (nursing or relevant field) preferred

• Certification: Basic Life Support (BLS) required

• Experience: Minimum 1 year Hospital Case Management required; Minimum of 2 years of hospital Case Management leadership experience preferred
