Job summary

Adventist Health Columbia Gorge in The Dalles, OR seeks a licensed Physical Therapist to plan and conduct segments of treatment programs that restore physical, social and mental function. You will co-lead in interdisciplinary teams to advance patient outcomes and collaborate across care disciplines.

Requires a Doctor of Physical Therapy degree with state licensure, CPR/BLS certification, and provides evaluations, individualized plans, and documentation.

Qualifications

• DPT degree required; licensure in the state of practice.
• Two years of PT experience preferred.
• CPR/BLS certification required.

Responsibilities

• Complete evaluations and address assessment areas needing attention.
• Develop and implement individualized treatment plans.
• Provide therapy interventions using established protocols.
• Create care and discharge plans and communicate them to the team.
• Complete clinical documentation within established deadlines.
• Supervise assigned assistants.
• Perform other duties as assigned.
