Adventist Health Columbia Gorge in The Dalles, OR seeks a licensed Physical Therapist to plan and conduct segments of treatment programs that restore physical, social and mental function. You will co-lead in interdisciplinary teams to advance patient outcomes and collaborate across care disciplines.

Requires a Doctor of Physical Therapy degree with state licensure, CPR/BLS certification, and provides evaluations, individualized plans, and documentation.

JOB DESCRIPTIONIn the heart of The Dalles, Oregon, Adventist Health Columbia Gorge has served the Columbia River Gorge community since 1901. We are comprised of a 49-bed hospital, various medical offices, home health, a birthing center and a vast scope of services located throughout The Dalles and the surrounding areas. Job Summary:

Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.

Education and Work Experience:

Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016; or foreign equivalent.

Two years' Physical Therapy experience: Preferred

Licenses/Certifications:

• Physical Therapy licensure in state of practice: Required
• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Required

Facility Specific License/Certifications:

• Basic Life Support (BLS) certification from approved vendor per AH policy: Required

Essential Functions:

• Completes evaluations to ensure quality standards and addresses assessment areas that require attention.
• Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients.
• Provides therapy interventions using appropriate treatment protocols and techniques.
• Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
• Completes comprehensive and accurate clinical documentation within established deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.
