Job summary

Empowerment Quality Care Services seeks a Qualified Professional to lead person-centered planning for children, adolescents, and families. You will coordinate multidisciplinary input and ensure linkage to clinically appropriate services, including psychological and psychiatric evaluations.

Qualifications

  • Must have 1 year documented experience working with children, adolescents, and families.
  • Licensed or certified in a human service profession (e.g., RN in NC) with MH/DD/SA experience.
  • Masters in Human Service field with 1 year supervised MH/DD/SA experience.
  • Bachelor's in Human Services with 2 years supervised MH/DD/SA experience.
  • Bachelor's in non-Human Services with 4 years supervised MH/DD/SA experience.

Responsibilities

  • Convene the person-centered planning process for the beneficiary.
  • Complete initial development and ongoing revision of the PCP.
  • Consult with medical and non-medical providers and input in PCP.
  • Arrange for psychological and psychiatric evaluations as needed.
  • Provide and coordinate behavioral health services and related interventions.
  • Monitor and document progress and effectiveness of PCP interventions.

Education

Bachelor's degree in Human Services with 2 years supervised MH/DD/SA experience
Bachelor's degree in non-Human Services field with 4 years supervised MH/DD/SA experience

Job description

Qualified Professional has responsibility for the following:

  • Convening the person-centered planning.
  • Completing the initial development and ongoing revision of the PCP.
  • Consulting with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school, DJJDP) providers, engaging community and natural supports, and including their input in the person-centered planning process.
  • Ensuring linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
  • Providing and coordinating behavioral health services and other interventions for the beneficiary or other family members with other licensed professionals and Child and Family Team members.
  • Monitoring and documenting the status of the beneficiary’s progress and the effectiveness of the strategies and interventions outlined in the PCP.

All IIH staff have responsibility for the following under the direction of the team leader:

  • Participating in the person-centered planning process.
  • Assisting with implementing a home-based behavioral support plan with the beneficiary and the beneficiary’s caregivers as indicated in the PCP.
  • Providing psychoeducation as indicated in the PCP.
  • Assisting the team leader in monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the PCP.
  • Assisting with crisis interventions.
  • Assisting the team leader in consulting with identified providers, engaging community and natural supports, and including their input in the person-centered planning process.

Requirements/Must Haves:

  • Must have one-year documented experience working with children, adolescents, and families.
  • An individual who holds a license, provisional license, or certificate issued by the governing board regulating a human service profession, including a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in MH/DD/SA with children, adolescents, and their families.
  • Graduate of a college or university with a Masters degree in a Human Service field and has one year of full-time, pre- or post-graduate degree accumulated supervised MH/DD/SA experiencewith children, adolescents, and their families, or a substance abuse professional who has one year of full-time, pre- or post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling.
  • Graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre- or post-bachelor's degree accumulated supervised MH/DD/SA experience withchildren, adolescents, and their families., or a substance abuse professional who has two years of full-time, pre- or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
  • Graduate of a college or university with a bachelor's degree in a field other than Human Services and has four years of full-time, pre- or post-bachelor's degree accumulated supervised MH/DD/SA experience withchildren, adolescents, and their families., or a substance abuse professional who has four years of full-time, pre- or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
