Community Care Network in Rutland, Vermont is seeking an Intensive Direct Support Professional (IDSP) to support individuals with dual diagnoses in daily living, community engagement, and transportation needs. The IDSP will work within the individual's home environment and collaborate with the care team to promote independence and safety.

The role requires a valid driver’s license, reliable transportation, and the ability to adapt to scheduling needs.

Qualifications

• Trauma informed care experience or familiarity preferred.
• Strong writing and organizational skills for documentation.
• Ability to maintain confidentiality and work independently.
• Flexibility and dependability in various scheduling scenarios.

Responsibilities

• Models, mentors, advocates, and motivates individuals to promote stability.
• Assist in planning and implementing individualized service agreements with the treatment team.
• Respond to medical or mental health emergencies and ensure safety.
• Utilize Therapeutic Options training for safety and well-being.
• Implement behavioral management and de-escalation strategies.
• Follow care procedures under RN supervision and seek nurse consultation as needed.
• Provide home and community-based support to individuals.
