Job summary

Bluebonnet Trails Community Services is seeking an Assertive Community Treatment Team Intensive Support Specialist to deliver person-centered, intensive support services to adults with substance use and a diagnosed severe and persistent mental health disorder. The caseload is small and services are integrated, community-based, combining psychiatric treatment, social services and rehabilitation.

Qualifications

• Bachelor’s degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education/intervention, or equivalent.
• Experience in behavioral healthcare, intensive community treatment, and substance use, abuse, and dependency treatment.
• Valid Texas Driver’s License and meet Bluebonnet Trails Community Services driving requirements; maintain approved driving status throughout employment.
• If degree is not in a Human Services field, a minimum of 24 college credits in a Human Services field is required.

Responsibilities

• Meet with individuals in homes, community centers, hospitals, crisis units, criminal justice settings, and BTCS clinics to provide specialized case management and rehabilitative skills training.
• Balance needs during crisis situations and prioritize safety.
• Engage in regular meetings and training opportunities to support recovery planning and teamwork.
• Work independently and on a team, coordinating services and advocating for clients.
