Job summary

Riverview Psychiatric Center is recruiting an Intensive Case Manager to serve in the social work department. You will deliver professional services in intensive case management for high-risk mental health clients, guiding their transition from hospital to the community and ensuring access to medical, psychiatric, rehabilitation, social, and educational supports.

You will monitor progress and coordination of services.

Qualifications

• Bachelor’s degree in social or behavioral sciences with two years of professional community mental health work.
• Master’s degree in social work may substitute for two years' experience.

Responsibilities

• Provide intensive case management for high-risk mental health clients.
• Assist clients in transitioning from hospital to community and accessing medical/psychiatric services.
• Monitor client integration progress and overall service delivery within the community-based system.
