Job summary

LifeSTEPS is seeking an Intensive Case Manager to support residents in Fremont, CA. The role involves on-site engagement across assigned properties, assessing needs in housing, finances and health, and coordinating tailored services to promote stability and independence.

The position requires a bachelor’s degree in social work, psychology or related field, plus two years of relevant experience and a valid California driver’s license.

Qualifications

• Bachelor’s degree in social work, psychology or related field.
• Two years of experience in a similar social services environment.
• Equivalent combinations of education and experience may substitute for the degree.
• Valid California driving license and reliable transportation required.

Responsibilities

• Provide intensive case management to individuals and families.
• Assess needs across housing, finances, employment, food, clothing and health.
• Coordinate and tailor services to identified needs at assigned properties.
• Spend the required contracted hours at each property and on-site as scheduled.
• Assist with job readiness activities, including resume writing and interviewing.
