Job summary

The Just One Project is seeking an Operation Home Case Manager to support individuals and families facing homelessness across Southern Nevada. You will manage a caseload of 20–25 clients, develop personalized housing and self-sufficiency plans, and connect them with community resources and landlords for stable housing.

You will advocate for benefits, coordinate with partner agencies, and conduct ongoing assessments and home visits.

Qualifications

• Bachelor's degree required in Social Work, Human Services, or related field.
• 3+ years nonprofit experience.
• 3+ years in case management, social services, or outreach.
• Experience with diverse populations, including vulnerable individuals.
• Certifications in case management and conflict resolution preferred.
• Experience managing a caseload and achieving outcomes in stability and self-sufficiency.
• Familiarity with housing services and wrap-around programs.
• Strong relationship-building while maintaining professional boundaries.
• Empathy, dignity, respect, client-centered approach.
• Strong problem-solving and conflict-resolution skills.
• Excellent organization and time-management; able to manage multiple priorities.
• Strong data-entry and documentation; proficient with HMIS and Salesforce.
• Knowledge of community resources and ability to connect clients to services.
• Coach clients in budgeting and resource navigation.
• Nevada Food Handler Safety Training Card within 30 days of hire.

Responsibilities

• Manage caseload of approximately 20–25 individuals and families.
• Assist clients to secure and maintain stable housing and connect with services.
• Conduct comprehensive assessments of client strengths, needs, goals, and barriers.
• Develop individualized action plans for housing, health, income, and stability.
• Advocate for benefits, entitlements, and community resources.
• Support housing searches and identify safe, affordable options.
• Liaise with landlords and assist with leases and utilities.
• Conduct monthly home visits and case reviews; monitor progress.
• Provide crisis support and safety planning when needed.
• Assist clients with budgeting, savings, and debt management.
• Collaborate with workforce, SOAR, nutrition education, housing, and other providers.
• Build relationships with partners and referral networks.
• Coordinate services in case conferences; adapt plans as needed.
• Maintain timely case documentation and HMIS/Salesforce entries.
