CASES in New York City is seeking an Intensive Case Manager to support justice-involved individuals re-entering the community. You will provide transitional case management, care coordination, and linkage to housing, health, and social services, with a focus on recovery-oriented, trauma-informed approaches.
This full-time role offers a 9am-5pm schedule, a caseload of 12, and collaboration with DOCCS, DHS, and OMH.

Job Title: Intensive Case Manager
Job Summary: CASES is a New York City agency working at the forefront of the intersection between behavioral health and involvement in the criminal justice system. Our mission is to increase public safety through innovative services that reduce crime and incarceration, improve behavioral health, promote recovery and rehabilitation, and create opportunities for success in the community. The Behavioral Health division consists of a continuum of services for justice-involved individuals with serious mental illness, including Certified Community Behavioral Health Clinic (CCBHC), health home care management (HHCM), forensic homeless intensive case management (ICM), assertive community treatment (ACT), and intensive mobile treatment (IMT). The Intensive Case Manager (ICM) is responsible for providing transitional case management services to individuals with serious mental illness who are re-entering from state prison to shelters in New York City.
Salary: The salary range for the Intensive Case Manager is $56,000 per year.
Shift Hours: The Intensive Case Manager is a full-time Monday - Friday from 9am to 5pm.
