Job Title: Intensive Case Manager
Job Summary:

CASES is a New York City agency working at the forefront of the intersection between behavioral health and involvement in the criminal justice system. Our mission is to increase public safety through innovative services that reduce crime and incarceration, improve behavioral health, promote recovery and rehabilitation, and create opportunities for success in the community.

The Behavioral Health division consists of a continuum of services for justice-involved individuals with serious mental illness, including Certified Community Behavioral Health Clinic (CCBHC), health home care management (HHCM), forensic homeless intensive case management (ICM), assertive community treatment (ACT), and intensive mobile treatment (IMT). The Intensive Case Manager (ICM) is responsible for providing transitional case management services to individuals with serious mental illness who are re-entering from state prison to shelters in New York City.

Salary:The salary range for the Intensive Case Manager is $56,000 per year.

Shift Hours:The Intensive Case Manager is a full-time Monday - Friday from 9am to 5pm.

Location Address: 2090 Adam Clayton Powel Jr Blvd, 4th Floor, New York, NY 10027.

Working environment:Flexible - (for roles that have day(s) that can be partially remote, e.g., being able to draft notes at home after going to the office/community in the morning).

What you will be doing:

  • Maintain a caseload of 12 clients and conduct at least 6 in-person services per month for all clients.
  • Provide essential transitional care coordination services, including pre-release contacts, day-of-release warm handoffs, assessments and service planning, and assistance with entitlements, housing, vocational, independent living skills, linkage to community supports (e.g., mental health, substance use, medical providers), advocacy, crisis response, and case conferences.
  • Provide services using a transitional practice framework (i.e., engagement, self-management skills, transfer of care) that is informed by critical time intervention methods.
  • Successfully connect clients to longer term community support services, such as health home care management or assertive community treatment.
  • Document all services in a comprehensive and timely manner in the case record, including assessments, service plans, progress notes, and discharge plans.
  • Complete required reports in the Child and Adult Integrated Reporting System (CAIRS) and the Assisted Outpatient Treatment (AOT) portal.
  • Attend relevant trainings, especially related to prison re-entry and behavioral health/criminal justice.
  • Collaborate professionally and effectively with stakeholders, such as the DOCCS/Parole, the Department of Homeless Services (DHS), and the Office of Mental Health (OMH).
  • Provide ICM services in a strengths-based, recovery-oriented, trauma-informed, and culturally humble manner.
  • Other duties as requested by the Team Leader or CASES senior management.

What we are looking for:

  • Bachelor’s Degree with a major or concentration in a relevant human services area
  • Five (5) years of experience in (a) providing direct services to individuals with serious mental illness (b) assisting individuals in their re-entry from incarceration to community settings, and (c) linking individuals with serious mental illness to a broad range of services essential to successful living in the community
  • Advanced clinical skills in the engagement of high-need individuals with serious mental illness, including motivational interviewing, suicide prevention, risk screening, trauma-informed care, person-centered care planning and interventions, and recovery-oriented approaches
  • Strong commitment to serving justice-involved individuals with behavioral health needs

Additional, preferred skills we are looking for but are not required:

  • Bilingual in Spanish/English
  • Individuals with lived experience in the areas of mental health, substance use, or criminal justice involvement are encouraged to apply
