Job Title: Intensive Care Manager I

Location: Manchester, TN
Schedule: Mon‑Fri, 8:00a.m.–4:30p.m.
Base Salary: $43,500 (plus incentive for language skills and behavioral health experience).
Mileage reimbursement available.

Job Summary

Provide continuous treatment team care management services to children and youth with serious emotional disturbance or severe mental illness, their families, and other team members’ consumers. Mentor new care managers on basic skills.

Responsibilities

• Conduct continual assessment of consumers’ needs and develop individualized service and crisis plans with measurable goals.
• Assist with referral and linkage to community resources; advocate for consumers as needed.
• Ensure consumers are seen upon discharge and meet Agency standards.
• Plan and coordinate daily psychiatric, medical, and environmental needs.
• Ensure efficient transition of consumers within the Agency and community.
• Meet productivity standards and regularly collaborate with Care Management Supervisor.
• Attend morning and weekly team meetings; follow‑up on plans developed.
• Facilitate education of consumers or collaterals about diagnosis and treatment options.
• Respond to urgent/crisis consumer needs with supervisor, crisis services, and clinic staff.
• Maintain relationships within family, school, juvenile justice, and other supportive agencies.
• Document promptly and maintain community relations.

Qualifications

• Bachelor’s Degree in counseling, psychology, social work, or other behavioral health field.
• Law‑related candidates must have fifteen semester hours of behavioral health coursework.
• Valid Tennessee Driver’s License.
• Acceptable Motor Vehicle Report, Criminal Background Investigation, Personal Automobile Insurance, and cell phone.
