Summary

We are currently hiring for an Intensive Care Management Coordinator-Behavioral Health to join BlueCross BlueShield of South Carolina. In this role, you will serve as an Intensive Case Manager (ICM) for members with specific conditions, assisting with case management activities/programs for our members.

Description

Location: This full-time position (40 hours/week) is Monday-Friday, 8:00am-5:00pm EST, fully remote, with required member-facing visits within South Carolina.

• Position offers mileage reimbursement after 50 miles with a rate of 0.7 per mile, travel limit of 3 hours one way.

What You Will Do:

• Conduct assessments, planning, coordination, monitoring, and advocacy for members. In-person contact required. Collaborate with members, families, and healthcare professionals. Ensure activities align with members' risk levels, needs, and goals. Assist in health management program implementation. Complete assessments in members' residences or similar environments. Participate in care meetings. Use data systems appropriately.
• Develop strategies to improve program effectiveness and participation. Maintain communication with providers, groups, and members.
• Analyze reports, adhere to regulations, and monitor compliance and success rates.
• Identify areas needing health management programs, monitor quality indicators, and develop educational materials. Perform additional duties as assigned.

Qualifications:

• Bachelor's degree in Nursing, Health Administration, Social Work, or related field with 45+ hours of relevant training.
• Experience working with individuals with health conditions (minimum 3 years, or 2 years if holding a master's degree).
• Skills in software, insurance understanding, quality improvement, independence, judgment, communication, critical thinking, confidentiality, and driving are required.
• Licenses: Active, unrestricted RN, LPN, or Social Work license as per role requirements.
