We are currently hiring for an Intensive Care Management Coordinator-Behavioral Health to join BlueCross BlueShield of South Carolina. In this role, you will serve as an Intensive Case Manager (ICM) for members with specific conditions, assisting with case management activities for members of BlueCross BlueShield of South Carolina, BlueChoice, and/or Healthy Blue Medicaid.

Location

This position is full-time (40 hours/week), Monday-Friday, from 8:00am-5:00pm EST, and is fully remote. It requires member-facing visits within South Carolina.

Responsibilities

1. Perform assessment, planning, coordination, monitoring, and advocacy for members, including in-person contact in members' residences or the least restrictive environments.
2. Coordinate health management activities and programs, ensuring they align with members' risk levels, needs, and preferences.
3. Conduct research and analyze reports, ensuring compliance with NCQA, state/federal regulations, and CMSA standards.
4. Develop strategies to enhance program effectiveness and participation, and create educational materials for members and providers.

Qualifications

• Bachelor's degree in a health-related field with relevant training hours, or equivalent experience as specified.
• Minimum of 3 years’ experience working with individuals with specific health conditions.
• Proficiency in Microsoft Office and understanding of insurance and health management programs.
• Active, unrestricted licensure (RN, LPN, Social Worker, or other relevant licenses) as specified.
