Job summary

LifeLong Medical Care seeks a licensed clinical social worker or licensed professional clinical counselor to join the East Oakland Health Center team in Oakland, CA. The role includes psychosocial assessments, short-term counseling, and collaboration with an interdisciplinary team to deliver comprehensive care.

Candidates must hold a California LCSW/LPCC license or an equivalent registration (ASW/AMFT/APCC).

Qualifications

• Must hold a California LCSW, LPCC license, or ACSW/APCC/AMFT registration.
• Experience providing mental health services to disadvantaged populations.
• Familiarity with evidence-based practices for treating depression and other disorders.

Responsibilities

• Provide psychosocial assessments and treatment plans for patients.
• Deliver short-term counseling and case management within clinic settings.
• Assist with crisis intervention and triage as needed.
• Coordinate follow-up assessments and prevention planning with other providers.
• Maintain clinical and administrative records and submit encounters weekly.
