Job summary

LifeLong Medical Care is seeking an Associate Social Worker (ASW), Associate Professional Clinical Counselor (APCC) or Associate Marriage and Family Therapist (AMFT) to join our Oakland, Berkeley and Pinole Health Centers. The role supports inter-disciplinary teams delivering psychosocial services to patients across primary care settings.

Qualifications

• Current and valid California LCSW, LPCC license, or ACSW, APCC or AMFT registration with the California Board of Behavioral Sciences.
• Knowledge and experience providing mental health services to disadvantaged populations.
• Familiarity with evidence-based practices for the treatment of depression and other mental disorders.
• Proficiency in applying clinical documentation and billing requirements.

Responsibilities

• Provides comprehensive psychosocial assessments, mental health diagnoses, treatment plans and implements short-term counseling.
• Provides short-term case management services for patients participating in treatment.
• Participates in crisis intervention and triage as needed.
• Maintains clinical and administrative records in accordance with contractual and billing requirements.
• Participates in weekly clinical meetings and quality improvement efforts.
