Job summary

Intermountain Health in Utah seeks a Mental Health Specialist (LCSW/LMFT/CMHC eligible) to join the Mental Health Integration team serving South Ogden, Roy, and McKay-Dee clinics. You will conduct psychosocial assessments, develop care plans with the primary care team, and provide solution-focused interventions to improve patient and family outcomes.

40-hour work week with a pay range of $52.12–$80.42 per hour; a $2,500 sign-on bonus is offered to external applicants; credentialing takes 60–90

Qualifications

• Master’s degree in social work or related field; licensure as required by state.
• Current state licensure (as required by state) for LCSW/LMFT/CMHC.
• Excellent written and verbal communication skills.
• Computer skills including email and word processing.

Responsibilities

• Assesses medical history, mental health history, and current presentation to understand needs.
• Diagnoses and treats mental, emotional, and behavioral disorders; may include psychotherapy.
• Develops a plan of care with input from patient, family, physicians, and healthcare team.
• Provides treatment based on patient needs and evidence-based interventions.
• Refers patients to community services, treatment programs, and specialists.
• Documents pertinent information regarding the plan of care.
• Consults with the multidisciplinary team to develop treatment and discharge plans.
