Job Summary: We are seeking a dedicated and compassionate Integrated Health Home (IHH) Nurse Care Manager to join our team. Key Responsibilities:
Care Coordination:
- Communicate with members and authorized persons in a culturally appropriate manner.
- Conduct initial contact for referrals and IHH intakes.
- Track caseload completion and funding, reporting to the IHH Supervisor as scheduled.
- Promote wellness through smoking cessation, substance use prevention, nutritional counseling, and physical activity.
Member Support:
- Identify and close gaps in care/services/resources.
- Prevent and manage chronic conditions using evidence-based practices.
- Provide linkage and support for community resources, insurance assistance, waiver services, peer advocacy groups, and more.
- Utilize motivational interviewing techniques.
- Offer comprehensive transitional care and education on preventing unnecessary emergency room use.
- Assist with transportation and housing resources.
- Monitor and advocate for member rights.
- Schedule and track appointments, conduct screenings, and encourage member engagement in goal development.
Caseload Management:
- Complete assigned contacts and outreach activities.
- Provide at least one IHH Core Service per month to caseload members.
- Document services within two business days.
- Ensure continuous funding and eligibility for IHH services.
- Conduct functional assessments and social histories.
- Organize and administer joint treatment planning.
- Monitor and adjust service plans as needed.
- Conduct claims-based monitoring and identify gaps in care.
- Coordinate access to services, supports, and resources.
- Monitor hospitalizations, ER visits, and follow-up appointments.
- Support members in achieving healthcare goals.
Team Collaboration:
- Attend and engage in QA, QI, huddles, staff meetings, and trainings.
- Follow the FRC IHH Manual to ensure standards are met.
- Partner with the IHH Nurse Care Manager and Peer Support Specialist.
- Participate in the on-call rotation for member consultation.
Qualifications:
- Registered Nurse (RN) with an Iowa license and at least three years of experience in mental health services, or a Bachelor of Science in Nursing (BSN) with an Iowa license and at least one year of experience.
- Valid driver's license and personal auto insurance.
- Willingness to use personal vehicle for work-related events and transport members.
- Ability to travel at least 30% of the time.
- Basic computer skills.
- Ability to lift a minimum of 40 pounds.
- Completion of new hire orientation and required trainings.
- Obtain and maintain CPR/First Aid Certification within 30 days of employment.
