Job summary

Commonwealth Care Alliance seeks an Integrated Care Clinical Manager in Massachusetts to manage complex care for dually-eligible enrollees. The RN will coordinate assessments, care plans, and multidisciplinary team activities, partnering with primary care and community resources to improve health outcomes.

This mobile role requires ongoing travel within the Massachusetts territory, adherence to NCQA standards, and strong communication across care teams.

Qualifications

• Registered Nurse with ability to independently serve people with complex medical, behavioral, and social needs.
• Experience in care coordination or case management preferred.
• Case management certification preferred.

Responsibilities

• Engage with enrollees in homes and community settings to establish a complex care management relationship.
• Coordinate with healthcare providers and community resources to ensure smooth care transitions.
• Lead interdisciplinary care team to develop holistic care plans addressing medical and non-medical needs.
• Oversee enrollee utilization of long-term services and supports to keep individuals in their chosen location.
• Educate members about Medicare and Medicaid benefits and services.
