Job summary

Paycom - ATS in Pittsburgh, PA is seeking a licensed mental health professional to provide patient-centered, trauma-informed care in a primary health setting for children, adolescents, and adults.

You will work with a multidisciplinary team, perform intake assessments, diagnose and refer, deliver evidence-based therapies (CBT, MI, DBT), and document progress in the EMR, while coordinating with PCPs and care coordinators.

Qualifications

• Completion of an approved Social Work, Clinical Counseling, or Clinical Psychology program.

Responsibilities

• Provide patient-centered, trauma-informed, and culturally competent mental health care in a primary health care setting for children, adolescents, and adults.
• Work collaboratively with a program psychiatrist/director, therapists, care coordinator, and PCPs.
• Conduct clinical intake assessments, screen for mental health and substance use disorders, and refer to outside services when indicated.
• Perform psychosocial needs assessments and connect patients to internal/external resources.
• Offer psychoeducation about mental health, substance use, and treatment options.
• Deliver evidence-based therapies (CBT, Behavioral Activation, MI, Prolonged Exposure, IPT, DBT skills).
• Monitor symptoms and communicate changes to the Mental Health Team.
• Facilitate access to in-clinic or outside resources or referrals.
• Engage in regular supervision with the Mental Health Team Manager/Director.
• Participate in weekly consultations focusing on high-risk patients.
• Use integrative care with all health staff.
• Engage patients in care and follow-up.
• Track outcomes using a registry and re-engage patients as needed.
• Document progress and recommendations in the EMR.
• Complete documentation and billing promptly.
• Develop and update treatment plans addressing whole-patient needs.
• Do step-down care and relapse-prevention planning as patients improve.
• Complete discharge summaries when necessary.
