The ideal candidate brings healthcare experience, care management skills, and strong communication abilities. Travel up to 40% may be required to visit members and providers across the service area.

Qualifications

• At least 2 years of health care experience, incl LTSS and care management.
• Licensure may be required by state contract; active/unrestricted license if applicable.
• Valid driver's license, reliable transportation, and adequate auto insurance.
• Experience with community resources and diverse populations.
• Strong verbal and written communication skills.
• Proficiency in MS Office and online portals.
• Experience with substance use disorder services in some states.

Responsibilities

• Completes comprehensive member assessments within timelines, including in-person visits.
• Facilitates waiver enrollment/disenrollment.
• Develops and implements care plans with members and care network.
• Monitors care plans and documents interventions.
• Promotes service integration across behavioral health and LTSS.
• Assesses medical necessity and authorizes waiver services.
• Coordinates ICT meetings for service approvals.
• Educates and motivates members using motivational interviewing.
• Identifies barriers to care and assists with psycho/social, financial, and medical obstacles.
• Travel up to 40% as required.
