Job summary

InReach is seeking a Full-time Tailored Care Manager in Charlotte, NC. The role focuses on whole-person care, planning, coordinating, and tracking to close gaps in care, with a hybrid schedule (in-office one day/week).

The candidate will engage with members, coordinate community referrals, and monitor transitions in care, primarily serving the I/DD population. Strong documentation and travel readiness are essential.

Qualifications

• Bachelor’s degree required.
• At least 2 years of experience with the I/DD population.
• Proficient in Microsoft Office and Excel.
• Valid driver's license and safe driving record.
• Access to reliable transportation and ability to travel to customers' homes.

Responsibilities

• Actively engages with members receiving care management services through assessment, care coordination, health promotion, and follow-up.
• Coordinates referrals to community resources and supports individuals in managing healthcare and personal needs.
• Coordinates follow-up after inpatient hospitalization or ER visit within 24 hours of discharge.
• Coordinates crisis response per InReach policies and participates in 24:7 crisis coverage.
• Participates in post-crisis debriefings and maintains thorough documentation.
• Establishes respectful relationships with individuals and care team members, ensuring clear communication.
• Maintains medical records, compliance, and data backups; keeps policies updated.
