Job summary

CNS Healthcare is seeking a Case Manager for the Assertive Community Treatment (ACT) program in Oakland County. The role focuses on community-based, home-based services to individuals with serious mental illness, promoting independence and community integration.

The position emphasizes collaboration with a multidisciplinary team, documentation accuracy, and trauma-informed care to support recovery and functional outcomes in a community setting.

Qualifications

• QMHP certification or eligibility
• Bachelor’s Degree in Social Work, Sociology, Counseling, Psychology, Criminal Justice, or other Human Services field
• Specialized training or one year of mental illness experience
• Valid Michigan Driver’s License and reliable transportation
• Proficient in Microsoft Office and EHRs

Responsibilities

• Provide community-based case management services and integrated services.
• Respond to needs and interests of individuals using assessment practices.
• Provide weekly community-based interventions and transportation assistance.
• Facilitate weekly groups for individuals served.
• Complete all required documentation accurately per CNS policy.
• Develop and implement Person-Centered Plans per established guidelines.
• Coordinate and monitor medications and healthcare providers.
• Engage in trauma-informed care and collaborative relationship building.
• Support individuals in achieving career and educational goals.
