Job summary

Kaiser Permanente in Stockton, CA is seeking a Consultant-Liaison to evaluate and diagnose Health Plan members seen in primary care who have mental disorders or behavioral conditions affecting their medical condition.

The role includes developing treatment plans, coordinating with physicians on psychotropic medications, and providing education and support to patients and families within a clinical team in a primary care setting.

Qualifications

• Master's degree in Social Work, Social Welfare from a clinical track, Clinical or Counseling Psychology or related field.
• Licensed Professional Clinical Counselor (California) OR Licensed Clinical Social Worker (California) OR Licensed Marriage and Family Therapist (California).
• National Provider Identifier required at hire.

Responsibilities

• Develop, implement, coordinate and evaluate treatment plans for referred members.
• Prepare intake evaluations and treatment plans, and maintain ongoing confidential progress records in accord with state and NCQA regulations.
• Provide education and support to members and their families to encourage compliance with medical and behavioral regimes.
• Co-manages medical conditions through nonpharmacologic and behavioral interventions to optimize physician time and utilization patterns.
• Collaborate with physicians in screening and evaluating patients for psychotropic medications within scope of practice.
• Provide consultation and training to APC team in mental health, behavioral medicine and health psychology.
• Provide consultation on program development and conduct program evaluation and outcome assessment as needed.
• Serve as liaison and refer to Psychiatry Department, Chemical Dependency Services, Behavioral Health Education and other Medical Teams.
