Job summary

Grand River Health in Rifle, CO is seeking an Integrated Behavioral Health Therapist to join a collaborative Primary Care team and provide brief behavioral health support within the primary care setting. You will help patients of all ages address concerns, strengthen coping skills, and improve self-management.

You will collaborate with providers and care team, respond to crises, and document in line with professional standards.

Qualifications

• Master’s degree in a recognized mental health profession with Colorado license in good standing.
• LCSW or LPC preferred; licensure must be obtained within 2 years.
• 2+ years of integrated care or clinical setting experience.
• Bilingual Spanish/English skills preferred.
• Current American Heart Association BLS certification within 3 months of initial employment.

Responsibilities

• Provide brief, targeted behavioral health interventions and short-term counseling using evidence-based approaches.
• Assess behavioral health needs, readiness for change, barriers to care, and treatment adherence while helping patients establish achievable goals.
• Support patients and families experiencing concerns such as grief, caregiver stress, substance use, new medical diagnoses, and other behavioral health conditions.
• Collaborate with providers, RN Case Managers, and the care team to coordinate treatment and resources.
• Respond to crisis situations and maintain timely, accurate documentation per standards.
• Perform a variety of tasks and other duties as assigned.
