Job summary

• Master’s degree in social work or related field.

• Excellent written and verbal communication skills.
• Computer Skills, e.g., email, word processing.

Responsibilities

• Conduct psychosocial assessments and provide behavioral health interventions.
• Develop discharge plans with the multi-disciplinary team to ensure continuity of care.
• Refer patients to community services and other mental health providers as needed.
• Educate and coordinate with patient care teams and outside agencies.
