Job summary

Role requires crisis intervention skills, strong documentation in EHR, and collaboration with the primary care team. Master's degree in Social Work/related field preferred; Michigan licensure or eligibility strongly preferred.

Qualifications

• Experience in casework principles including diagnosis, assessment, crisis intervention.
• Knowledge of Integrated Behavioral Health practices in primary care.
• Ability to communicate with diverse populations.
• Membership in professional organizations (e.g., APA, NASW) preferred.

Responsibilities

• Assess clinical status of patients referred by the primary care provider.
• Provide brief behavioral interventions using evidence-based techniques.
• Document treatment progress in EHR and coordinate with medical providers and the care team.
• Provide crisis intervention and short-term consultation to the primary care team.
