• Graduate of an accredited Medical Assistant program.
• Experience in behavioral health, primary care, community health, or integrated healthcare preferred.

Responsibilities

• Collect and document vital signs, health histories, medications, and screenings.
• Gather medical records, lab results, referrals, and follow-up information from hospitals, primary care providers, specialists, pharmacies, and other healthcare partners.
• Identify missing or overdue health information, preventive screenings, and follow-up needs.
• Track outstanding items and ensure unresolved needs reach the clinical team.
• Support quality initiatives, population health activities, and care coordination.
