Chiricahua Community Health Centers, Inc. seeks a licensed Behavioral Health Counselor to provide comprehensive counseling services, assess and diagnose, develop treatment plans, and deliver individual, family, and group therapy within integrated care teams.

Collaborate with primary care and psychiatric providers, maintain documentation in the EHR, and adhere to trauma-informed, culturally competent care. Arizona-licensed clinicians welcome bilinguals.

Qualifikationen

• Master's degree in counseling, social work, marriage and family therapy, or a closely related behavioral health field from an accredited program.
• Current, active license to provide behavioral health counseling in Arizona (LPC/LMFT/LCSW/LIAC or another independent license approved by CCHCI).
• Completes continuing education and maintains licensure in good standing with the Arizona Board of Behavioral Health Examiners.
• Valid Driver's License may be required for mileage reimbursement and travel.
• Fingerprint Clearance Card may be required.

Aufgaben

• Provides and manages care to children (ages 6-11), adolescents (ages 12-17), and adults (ages 13+).
• Diagnoses behavioral health and substance use disorders per DSM-5/ICD-10 standards.
• Conducts comprehensive intake assessments to inform the plan of care.
• Develops individualized, measurable treatment plans with clients.
• Provides individual, family, and/or group psychotherapy using evidence-based modalities.
• Monitors progress and adapts interventions as needed.
• Documents clinical notes timely in the EHR.
• Assesses risk and provides safety planning for urgent needs.
• Participates in multidisciplinary care team meetings and coordination with providers.
