Centerpoint Health is seeking a Licensed Practical Nurse (LPN) to provide clinical support to our health care providers and assist with patient care at our health center locations in Franklin, and Middletown, Ohio.POSITION SUMMARYThe Licensed Practical Nurse works under the direction of a physician, advanced practice provider, or registered nurse. The LPN provides nursing care, patient education, care coordination, and clinical support in accordance with the LPN scope of practice, established procedures, and Centerpoint Health policies.JOB DUTIESObtain and document patient health information in the electronic medical record.Collect and record basic clinical information and communicate patient concerns to the appropriate provider.Assist with patient assessments and care planning as directed.Provide patient and family education within the LPN scope of practice.Provide education regarding medications, diet, exercise, diagnostic testing, and follow-up care.Communicate patient needs and clinical information to physicians, advanced practice providers, registered nurses, and other members of the care team.Collaborate with the health care team to support quality and continuity of patient care.Follow established Centerpoint Health clinical policies, procedures, and protocols.Assist with patient scheduling and daily clinical workflow.Assist with addressing patient and staff concerns and communicate concerns to the appropriate supervisor.Support efficient provider schedules and patient flow while maintaining quality patient care.Review the after-visit summary with patients and provide appropriate follow-up instructions.Carry out provider orders within the LPN scope of practice and established protocols.Process medication refill requests according to established procedures.Administer medications and treatments as ordered and within the LPN scope of practice.Complete laboratory requisitions and assist with appropriate specimen collection and processing.Complete required laboratory and insurance documentation.Assist with insurance authorizations, referrals, diagnostic scheduling, and specialty appointments.Assist patients with scheduling services ordered by their provider.Maintain accurate and timely documentation in the electronic medical record.Perform other duties as assigned.EDUCATIONLicensed Practical NurseLICENSURECurrent unrestricted Ohio LPN license required.EXPERIENCEThree years of experience as an LPN providing direct patient care is preferred.Primary care experience preferred.Experience in a community health center or Patient-Centered Medical Home preferred.Customer service experience required.SKILLS AND QUALIFICATIONSStrong attention to detail.Ability to follow established procedures and instructions.Ability to work independently within the LPN scope of practice.Strong communication and interpersonal skills.Ability to work effectively with physicians, advanced practice providers, registered nurses, medical assistants, and other health care team members.Ability to manage multiple priorities in a busy health care environment.Knowledge of the roles and responsibilities of members of a clinical care team.Ability to support efficient delivery of patient services.Basic computer skills and experience with electronic medical records preferred.Blood-Borne Pathogens Exposure: Category 1PHYSICAL REQUIREMENTSThis position may require the ability to lift up to 25 pounds and perform activities including bending, walking, navigating steps, pushing, pulling, and squatting.
