The role requires a Master’s degree in Counseling, Social Work or Doctorate in Psychology, Arizona licensure, and experience in healthcare settings.

Qualifications

• Master’s degree in Counseling, Social Work or Doctorate in Psychology
• Licensed in Arizona (LPC/LCSW or Psychologist) required
• Minimum 2 years in healthcare settings
• Bilingual English/Spanish preferred

Responsibilities

• Conduct comprehensive psychosocial assessments within the Primary Care Behavioral Health model.
• Coordinate care with a multidisciplinary team to develop patient-centered plans.
• Provide brief interventions and short-term counseling for mental health concerns.
• Deliver crisis intervention, risk assessment, and safety planning.
• Document all encounters in the electronic health record per policy.
