Legacy Community Health in Houston is seeking a Behavioral Health Consultant (LPC, LCSW, or LMFT) to provide integrated behavioral health services within a primary care setting. The role supports adults, adolescents, and children with screenings, brief interventions, and counseling in a collaborative care model.

You will work a schedule from 8AM to 4PM, collaborating with physicians and other providers to implement client-centered treatment plans and ensure access to community resources.

Competenze

• Master’s degree from an accredited college or university in the social or behavioral sciences.
• Current Texas license (LCSW, LPC, or LMFT) in good standing with the credentialing board.
• Provider shall participate in Medicare/Medicaid programs and other reimbursements as requested by Legacy.

Mansioni

• Screen new clients identified at risk in the primary care setting and provide referrals as needed.
• Administer psychosocial assessments and crisis intervention.
• Provide individual brief interventions to adults, adolescents, and children.
• Provide direct clinical services including clinical evaluations and counseling.
• Review client records and write evaluation reports.
• Coordinate consultation and services with community agencies and health providers.
• Document all encounters and complete records within 3 working days.
• Perform billing activities according to agency procedures.
