Qualifications

• Master’s degree from an accredited graduate school of social work or related field from a regionally accredited university.
• Maintain a current, active license with the State of Alaska as a Licensed Clinical Social Worker.
• Must maintain current BLS/CPR certification.

Responsibilities

• Provide care to adults, children, and adolescents using a biopsychosocial-spiritual-cultural assessment.
• Screen, assess, triage and manage patients in a primary care setting.
• Coordinate with primary care providers to integrate behavioral health plans.
• Document in the EHR and coordinate referrals to specialty care as needed.
• Teach patients and families about care and prevention techniques.
