Job summary

KC CARE seeks a Behavioral Health Consultant to provide screenings, assessments, and brief therapeutic interventions in a primary care setting. You will integrate physical and behavioral health within the Patient-Centered Medical Home model.

Requires a Master’s degree in social work, psychology or counseling and Missouri licensure (Ph.D., LPC or LCSW). Minimum six months of relevant experience; travel to community sites may be required.

Qualifications

• Master's degree required in social work, psychology or counseling.
• Licensed in Missouri as a Ph.D., LPC or LCSW.
• Six months of directly related experience preferred.

Responsibilities

• Provides on-site behavioral health screenings, assessments, consultations, and referral in a primary care setting.
• Provides brief and short-term behavioral intervention and crisis intervention as needed.
• Documents health records in the EMR and follows up on treatment effectiveness.
