Zusammenfassung

Family First Health is seeking an independently licensed LCSW, LPC, or LMFT to join our George Street Center as a Part-Time Behavioral Health Consultant within an integrated primary care team in York, PA.

You will assess, intervene briefly, and develop care plans while collaborating with medical providers, monitoring progress with screening tools, and connecting patients to specialty services when needed.

Qualifikationen

• Must hold a current, unrestricted Pennsylvania license as an LCSW, LPC, or LMFT.
• Experience in integrated primary care and interdisciplinary teamwork preferred.

Aufgaben

• Provide brief behavioral health assessment and brief interventions in primary care.
• Collaborate with Primary Care Providers and an interdisciplinary care team.
• Develop patient-centered treatment and care plans.
• Use screening and measurement tools to monitor progress.
• Provide consultation and behavioral health education to patients and staff.
• Coordinate follow-up care and referrals to specialty services when needed.
• Document care in the electronic health record and communicate with the treatment team.
