Job summary

Family First Health seeks an independently licensed LCSW, LPC, or LMFT to join the George Street Center as a Behavioral Health Consultant. You’ll work with medical providers to address behavioral health needs within an integrated primary care team.

You’ll conduct brief assessments, provide evidence-based interventions, and develop care plans, coordinating with the care team and using EHR documentation. This role emphasizes team-based, accessible behavioral health within healthcare.

Qualifications

• Must hold a current, unrestricted Pennsylvania license as an LCSW, LPC, or LMFT.
• Experience in integrated primary care or similar interdisciplinary clinical settings preferred.
• Ability to provide brief, evidence-based interventions in a primary care setting.

Responsibilities

• Provide brief behavioral health assessment and evidence-based interventions within the primary care setting.
• Work collaboratively with Primary Care Providers and other members of an interdisciplinary care team.
• Develop patient-centered treatment and care plans.
• Use screening and measurement tools to monitor patient progress and guide care.
• Provide consultation and behavioral health education to patients and care team members.
• Utilize Motivational Interviewing to support meaningful behavioral change.
• Assess behavioral health and safety concerns and coordinate follow-up care.
• Connect patients with specialty behavioral health and community resources when appropriate.
• Document clinical care and communicate with the treatment team through the electronic health record.

Family First Health is seeking an independently licensed LCSW, LPC, or LMFT to join our George Street Center team as a Behavioral Health Consultant.This is not a traditional outpatient therapy role.
