Minimum requirements include a graduate degree in a mental health field and an active WA license (LICSW/LMFT/LMHC) with at least two years of experience in health education; English

Qualifications

• Graduate degree in mental health field and active licensure in WA State (LICSW/LMFT/LMHC).
• Two (2) years of experience providing individual or community health education.

Responsibilities

• Provide integrated behavioral health care within a primary care setting under the PCBH model.
• Collaborate as part of a care team including primary care, behavioral health, and other medical staff to improve patient health outcomes.
